What's your name? = Anong pangalan mo? (informal)
                                 Ano po ang pangalan nila? (formal)



My name is ________    = Ang pangalan ko ay ________.

Example:
Ang pangalan ko ay John.
(My name is John.)

Or you can also introduce yourself by saying:

I am ________ = Ako si ________ (informal)
                          Ako po si  ________  (formal) 

Example:
Ako si John.
(I am John.)
